Why Does It Still Feel Hard?

Ever leave a session wondering why it still feels hard, despite all your experience, training, and effort?

You're not alone.

Most therapists enter the profession believing that experience will eventually make the difficult cases easier. Yet many of us find ourselves years into practice still questioning why some children remain stuck, why progress can be slow, and why certain situations continue to challenge us.

The truth is that paediatric practice has become increasingly complex.

Children are presenting with more diverse needs. Families are carrying significant stress. Schools are stretched. Funding systems are changing. And despite our best clinical reasoning, evidence-based interventions, and strong therapeutic relationships, outcomes do not always unfold as quickly as we hope.

That does not mean you are failing!

Sometimes the challenge is not about finding a better strategy. Sometimes it is about understanding the many factors influencing a child's participation, regulation, relationships, and capacity to engage.

As therapists, we can become so focused on outcomes that we overlook the progress happening beneath the surface:

  • A parent who feels more confident.

  • A child who trusts us enough to try.

  • A teacher who begins to see behaviour differently.

  • A family who feels understood for the first time.

These changes matter. They often form the foundation upon which future growth is built.

Research consistently highlights the importance of the therapeutic relationship as a contributor to positive outcomes. While interventions matter, the quality of the alliance between therapist, child, and family plays a significant role in supporting meaningful change.

Perhaps the question is not:

"Why isn't this working?"

But rather:

"What might I be missing?"

This is one of the reasons I am passionate about supervision.

Good supervision is not about being told what to do. It is a space to step back, reflect, untangle complexity, strengthen clinical reasoning, and explore the factors that may be influencing a child's progress. Sometimes a fresh perspective helps us see possibilities that are difficult to identify when we are in the middle of a challenging case.

After more than 30 years in paediatric practice, I still value opportunities to reflect, learn, and think alongside colleagues.  I also continue to benefit from my own supervisor! (My grandmother told me "you can learn something new every day - so i continue to try to do that.)

Because the goal is not to become the therapist who has all the answers.

The goal is to become the therapist who keeps asking the right questions.

And sometimes, that makes all the difference... even if it is just for one child.
