Why escalation moments are so difficult in paediatric OT

Many therapists are taught tools and strategies.

But far fewer are taught how to decide what to do in the moment when a child escalates.

When regulation breaks down, therapists often find themselves wondering:

• Is this behaviour or nervous system overload?
• Should I push through or pause?
• Why do strategies work sometimes but fail other times?
• How do I explain what I did clinically in my notes?

Without a clear framework, these moments can feel uncertain.

And documentation often ends up sounding vague:

"Sensory strategies were used."

But clinical reasoning deserves to be clearer than that.
